  If you have an appointment date
Send in the information for the five weeks before the appointment.
  If you don’t have an appointment yet
Upload the documents as soon as you receive the appointment.
  If you are receiving benefits or pension
1.	A photograph of the whole letter notifying you of the benefit or pension; and 

2.	A copy of your bank statement showing the payment going in during the month before the appointment.  

Please make sure we can read your name and the payment amount of your benefit or tax credit.  

  If you are on Universal Credit
1. A screenshot from your online Universal Credit Account showing the most recent payment; and 

2.	A copy of your bank statement showing the payment going in during the month before the appointment.

  If you are living with a family member or friend 
A family support form signed by the person you are living with. (Download form here)
  If you are receiving financial support from a family member or friend (this includes paying bills on your behalf or providing food)
A family support form signed by each person providing you with support. (Download form here)
  If you are working
All wage slips covering the five week period before your appointment.  If you are receiving any other form of income please see the above relevant point.
  If you have a partner you live with
All wage slips covering the five week period before your appointment.  If they are receiving any other form of income please see the above relevant point.
   If you are renting
A copy of your rental agreement and housing benefit letter if appropriate. 
   If you are paying a mortgage
A copy of mortgage statement or bank statement showing monthly payments.
